Cord Blood Acid-Base Values of the Normal Newborn Infants in Uncomplicated Term Cesarean Sections






Abstract
Gord blood acid-base values were determined from 77 normal infants born after uncomplicated 67 antepartum and 10 intrapartum cesarean sections at 37 to 41 weeks' gestation.
All infants had Apgar score¡Ã7 at 1 minute.
Antepartum umbilical arterial mean values¡¾SD for acid-base indexes were: pH, 7.27¡¾0.06; PO2, 23.9¡¾7.5mmHg; PCO2, 50.7¡¾8.6mmHg; bicarbonate, 22.4¡¾3.4mEq/L; BE, -4.8¡¾3.5mEq/L.
Antepartum umbilical venous mean values¡¾SD were: PH, 7.30¡¾0.06; PO2, 33.7¡¾8.8mmHg; PCO2, 44.9¡¾6.8mmHg; bicarbonate, 21.9¡¾3.9/mEq/L; BE, -4.7¡¾3.3mEq/L.
Intrapartum umbilical arterial mean values¡¾SD were: PH, 7.27¡¾0.48; PO2, 26.5¡¾11.1mmHg; PCO2, 47.8¡¾11.8 mmHg; bicarbonate, 22.5¡¾3.7/mEq/L; BE, -4.9¡¾3.0mEq/L.
Intrapartum umbilical venous mean values¡¾SD were: PH, 7.31¡¾0.05; PO2, 34.8¡¾10.2mmHg; PCO2, 44.9¡¾16.3mmHg; bicarbonate, 24.4¡¾9.6/mEq/L; BE, -4.7¡¾2.2mEq/L.
There was no significant difference in the umbilical cord acid-base values between antepartum and intrapartum cesarean section, nor was any significant difference found in the umbilical acid-base values of 10 cases of intrapartum cesarean section
according to different methods of anesthesia, or in the acid-base values between various indications for cesarean section. However, in the 67 cases of antepartum cesarean section who underwent general anesthesia, the umbilical arterial PO2 was
significantly low, and the umbilical arterial and venous base excess was siginificantly high.
Normal values of cord blood acid-base in antepartum and intrapartum cesarean section should be used to define the basic limits for the resuscitation of sick-newborn infant as well as for the evaluation of neonatal outcome.
As there were only a few number of cases in this study, it is thought that more cases should be accumulated and acid-base values analyzed, especially in intrapartum cesarean sections.
